This event is being organised by the West Midlands Patient Safety Collaborative (PSC) to facilitate your progression in the implementation of Patient Safety Incident Response Framework (PSIRF) and bring PSIRF leaders together to collaborate, share and learn with other organisations within the region.

PSIRF represents a significant shift in the way the NHS responds to patient safety incidents and is a major step towards establishing a safety management system across the NHS. This transformative cultural change is a key part of the NHS patient safety strategy and PSIRF supports the development and maintenance of an effective patient safety incident response system that integrates four key aims:

  • Compassionate engagement and involvement of those affected by patient safety incidents
  • Application of a range of system based approached to learning from patient safety incidents
  • Considered and proportionate responses to patient safety incidents
  • Supportive oversight focused on strengthening response system functioning and improvement

The event is free to attend, all we ask is for you to bring your ideas and enthusiasm to share good practice and use the event as an opportunity to gain knowledge, insight and network.

Who should attend?

We are inviting integrated care systems and provider trusts from the following regions: Black Country; Birmingham and Solihull; Coventry and Warwickshire; Staffordshire and Stoke-on-Trent; Herefordshire and Worcestershire and Shropshire Telford and Wrekin.

Participants from acute, community, mental health and ambulance providers from across the region, are invited to take up to 2 places each at this event. We are particularly keen to see the following roles represented at this event.
